Item Awareness and attitudes among nursing officers of National Institute of Mental Health, regarding the COVID 19; Descriptive cross-sectional study(Faculty of Medicine, University of Kelaniya, Sri Lanka, 2021) Dharamarathna, N.D.; Samarasekara, D.; Madubhashin, D.; Rupasena, S.; Rajapaksha, N.; Sansala, T.Introduction: Individuals who seek inpatient hospital care due to COVID 19 infection are drastically increasing and nurses are crucial in caring for patients. Proper awareness and positive attitudes are the key pillars for pertinent caring of patients with COVID 19. Objectives: The study aimed to evaluate the awareness and attitudes towards COVID- 19 among nursing officers of National Institute of Mental Health (NIMH), Sri Lanka. Methods: A descriptive cross-sectional study was conducted among randomly selected 201 nurses of NIMH. A pretested self-administered questionnaire was used to collect data. Data were analysed using descriptive statistics and Pearson chi-square test. Overall awareness was categorised using modified Bloom’s cut-off point as good (80%-100%), moderate (50%-79%), and poor (< 50%). Results: The majority were female (92%) with a mean age of 31±6 years. Of participants, 40% reported a history of exposure to COVID 19 while 27% had quarantine. Nearly half of the participants (52%) had moderate awareness with a mean of 70±14 (range 20-90). Referring the COVID 19 guidelines (p=0.001) and working experience (p=0.04) was significantly associated with the awareness. The mean value of the attitudes was 61 ±6 (range 46-80) and 63% (n=127) had a neutral attitude towards COVID-19. Working in a medical ward (p=0.01) or COVID suspected ward (p=0.005) and experience of quarantine (p=0.01) were significantly associated with attitudes. Conclusions: Overall awareness and attitudes were at a moderate level among most. It is necessary to take measures to enhance the knowledge and attitudes towards COVID 19 among nurses.Item Disease control strategies for extending storage life of papaya (Carica papaya), cultivars ‘Red Lady’ and ‘Rathna’(2012) Abeywickrama, K.P.; Wijerathna, C.; Rajapaksha, N.; Sarananda, K.; Kannangara, S.Anthracnose (Colletotrichum gloeosporioides), stem-end rot (Lasiodiplodia theobromae) and Phomopsis rot (Phomopsis caricae-papayae) are major post-harvest diseases in papaya resulting in relatively high postharvest losses, up to 45%, in Sri Lanka. The current consumer preference is for tropical fruits free of synthetic pesticides. The objectives of the present investigation were to identify major post-harvest pathogens in papaya fruits, test in vitro fungitoxic effect of basil oil (Ocimum basillicum) and alum (sodium aluminium sulphate) on major fungal pathogens of papaya fruit and develop a relatively safe, disease control strategy. Alum (1% w/v) and basil oil (0.12– 0.16% v/v) in a liquid bioassay, indicated fungistatic and fungicidal efficacy on C. gloeosporioides, L. theobromae and P. caricae-papayae isolated from „Red Lady‟ and „Rathna‟ cultivars of papaya. In vivo investigation of fruits of both papaya cultivars harvested from fruit plantations in Sri Lanka („Red Lady‟–Yatawarawatta plantation, Kurunegala, „Rathna‟ – CIC plantation, Kekirawa and the Agricultural farm, Polonnaruwa) at 25% maturity level indicated that papaya washed in 1% (w/v) alum and subsequently sprayed with an emulsion solution of 0.16% (v/v) basil oil and enclosed in styrofoam sleeves could be stored for 14 days at 12–14 oC. The visual quality rating (VQR) was high (11–13), shriveling and „green islands‟ were low on fruits with zero disease severity where results were statistically similar to 500 mg/L Bavistin treatment. Physicochemical properties assessed were unaffected by the treatments and were comparable to the controls washed only in water. The organoleptic properties were slightly low in fruits subjected to cold temperature compared to fruits subjected to ambient temperature. No residues of basil oil could be detected on fruit peel by gas chromatography at the end of the experimental period of 14 days. The treatment strategy developed could be adopted for sea shipment or temperature-regulated supermarket storage of papaya fruits where storage life could be lengthened up to 14 days.Item Is being barefoot, wearing shoes and physical activity associated with knee osteoarthritis pain flares? Data from a usually barefoot Sri Lankan cohort(Oxford, 2021) Atukorala, I.; Pathmeswaran, A.; Batuwita, N.; Rajapaksha, N.; Ratnasiri, V.; Wijayaratne, L.; de Silva, M.; Chang, T.; Zhang, Y.; Hunter, D.J.AIM: To identify the association between hours of being barefoot/wearing footwear, physical activity (PA) and knee osteoarthritis pain flares (KOAF). METHODS: Persons with a diagnosis of knee osteoarthritis, who reported previous KOAF, were followed up in a 3 months long telephone-based case-crossover study. Exposures to risk factors were assessed every 10 days and whenever the participants experienced a KOAF. Conditional logistic regression examined associations of KOAF with following: hours of being barefoot/using footwear and PA performed (P < .05). RESULTS: There were 260 persons recruited, of whom 183 continued longitudinal follow up. Of them, 120 persons had at least one valid KOAF and control period. Participants were female (90%) with mean (SD) age and body mass index of 59.9 (7.0) years, 28.0 (5.0) kg/m2 respectively. Participants were barefoot for a mean duration of 12.7 hours (SD 4.6) and used footwear for 5.1 (SD 4.7) hours daily; 99% wore heel heights <2.5 cm. Duration of being barefoot, 1 and 2 days before, demonstrated reduced multivariate odds of KOAF (odds ratio [OR] = 0.85; 95% CI 0.80-0.90). Moderate PA performed 1, 2 days prior was associated with a significantly increased risk of KOAF (multivariate OR 4.29; 2.52-7.30 and OR 3.36; 2.01-5.61). Similarly, hours of using footwear 1 and 2 days before flare demonstrated increased odds of KOAF (OR 1.15; 1.07-1.23 and 1.10; 1.03-1.18). CONCLUSIONS: Increased duration of being barefoot 1 to 2 days before is associated with reduced risk of KOAF. Performing moderate PA 1 to 2 days before was associated with an increased risk of KOAF. KEYWORDS: knee osteoarthritis pain.Item Language Skills of 2-5 Year Old Children with Profound Hearing Loss Undergoing Auditory-Oral Education Compared to Typically-Developing Peers.(Faculty of Medicine, University of Kelaniya, 2014) Waidyasekara, C.; Rajapaksha, N.; Herath, K.; Hettiarachchi, S.Hearing impairment is the most frequent sensory defect that leads to communication difficulties and critical to speech and language development, communication, and learning. The objectives were to compare the language skills of children with profound hearing loss using digital amplification and undergoing intensive Auditory Oral education with typically developing peers and to determine the correlation between the time period of receiving Auditory Oral education and language outcomes. Twenty-two children with profound hearing loss using bilateral Behind- The- Ear hearing devices, were selected from an Auditory Oral pre-school. 44 children whose age, gender and geographical area matched were selected from mainstream pre-schools. All participants were assessed using the informal assessment tool. Median scores of receptive vocabulary skills are higher than scores of expressive vocabulary skills in both groups. The difference was statistically significant between the two groups. Receptive vocabulary, understanding of quality, understanding of spatial concept, understand of sequencing concept, understand of sentence structure (syntax) and integrative thinking revealed significant (p<0.05) for each age range 2;0 – 2;11, 3;0 – 3;11 and 4;0 – 5;0. There is a significant difference in seventeen out of twenty linguistic components between the two groups (p<0.05). Only three categories; initiating (p=0.157), producing syllables (p=0.157) and expressing negations (p=0.157) did not show significance. There is a significant (p<0.01) correlation among the time period of having AO education and language skills. The study concluded that there is a significant difference in language skills compared to the typically developing peers. Increased language skills were observed with the early attendance of Auditory Oral education and with appropriate hearing aid. Hence, the children with hearing loss need effective intervention to enhance their language skills in spoken language.Item Molecular diagnosis of Williams Buren syndrome in a cohort of Sri Lankan patients(Sri Lanka Medical Association, 2012) Ranaweera, D.M.; de Silva, D.; Samarasinghe, D.; Perera, S.; Rajapaksha, N.; Chandrasekharan, N.V.INTRODUCTION: Williams Bueren Syndrome (WBS) is a common genetic cause of congenital heart defects associated with developmental delay, hypercalcaemia and characteristic facial dysmorphism. It is caused by a 1.5 to 1.8 Mb deletion of chromosome 7qll.23 involving the loss of around 23 genes including the elastin (ELN) gene. This study reports the development of a semi quantitative PCR method to diagnose WBS. AIMS: To establish a molecular diagnostic test for WBS and determine the frequency of ELN deletions among clinically suspected cases. METHODS: Sixteen suspected WBS cases identified by two paediatric cardiologists were recruited following ethical clearance and informed consent. DNA was extracted and dosage analysis was carried out using semi-quantitative PCR. In a multiplex PCR reaction normal (N), positive control (with a confirmed deletion) and patients' (PJ DNA was amplified using 2 primer pairs which amplified regions within the ELN gene and the CFTR gene on chromosome 7 but outside the deleted region. Following agarose gel electrophoresis, the amplified products were quantified. A ratio of P:N of 0.5 indicated the presence of a deletion while a ratio of 1 indicated the absence of a deletion. RESULTS: Among sixteen suspected cases, 12 (75%) had an ELN gene deletion while 4 cases did not. CONCLUSIONS: This semi-quantitative PCR method was able to distinguish ELN deleted cases from the non deleted ones. The preliminary data supports this as a useful diagnostic test for WBS but validation is required before its clinical use.
