    Efficacy of Ayurvedic formula mentioned in Susrutasamhitha in the management of Kalithya

    Hair loss becomes one of the common problems in society. Falling hair is one of the diseases of head region, known as “Kalithya” in Ayurveda. There are varieties of reasons for hair loss, including autoimmune diseases, stress, poor nutrition, some medications, mineral deficiency, pollution, severe illness, excessive styling, pregnancy, hereditary and even poor hair care, while wearing helmets and caps can also increase the risk of hair loss. Today more and more people are turning toward the Ayurveda to find a solution for hair loss. Ayurveda believes that hair fall control is very much associated with the body type and also the stability of both body and mind structure of the person. To overcome Kalithya there are many treatments modalities described in Ayurveda texts, like Shirolepa, Shiroabyanga, Shirodhara, Shirovasthi etc. In this study, shirolepa was considered as the treatment modality. Application of medicated paste on the scalp is known as Shirolepa. Most of the shiro lepa are not scientifically evaluated. This formula is one of them mentioned in Susrutasamhitha. Equal quantities of Karanda (Pongamia pinnata) leaves, juice of Kapitta (Limonia acidissima L.) and Kasisa purified using Bhrungaraja (Eclipta alba) decoction were used to make the herbal paste. In the clinical study 20 patients were selected in age between 20-50 years, irrespective of their religion, occupation and habits. They were randomly divided in to two groups. Group A was treated with shiro lepa and group B was treated with Brungamalaka Thaila (traditional effective Thaila for scalp). All of them completed the treatments accurately and the follow up study continued up to one month after treatments. After considering the results, it can be concluded that herbal paste is more effective in fallen hair, itching condition, hair dandruff and hair color except regrowth of the hair

    Evaluation of the anti-bacterial effect of traditional Lodhradi lepa and Lodhradi cream against the laboratory specimen of staphylococcus aureus: a laboratory experiment

    Acne vulgaris is a chronic disease which destroys the beauty and cause disfigurement of the face especially in the adolescence. Acne vulgaris is known as Mukhadushika or Yuvana Pidaka in Ayurveda. Acne vulgaris, secondary infected by Staphylococcus aureus. Lodhradi lepa is an external application for Yuvana Pidaka in Ayurveda. The study was to test the anti-bacterial activity of the traditional Lodhradi lepa and Lodhradi cream against the Staphylococcus aureus. The anti-bacterial sensitivity test was done on well diffusion method with freshly prepared drugs. In trial one it was tested with freshly prepared drugs and the second trial was done with two different concentrations of freshly prepared drugs. In both trials the zone diameters were among 00-12mm and as the diameters were not among the resistant or susceptible level Staphylococcus aureus is not susceptible to these external ointments. Erythromycin the positive one was highly susceptible. Staphylococcus aureus was not susceptible for the traditional Lodhradi lepa and Lodhradi cream. The traditional Lodhradi lepa and Lodhradi cream would be used in acne vulgaris to reduce inflammation like symptoms due to its pacifying properties.

    A survey study to assess the knowledge and attitudes towards sexually transmitted diseases among late adolescents

    There is an ongoing change in the sexual behaviour patterns of adolescents all over the world. Therefore, early implementation of factors related to AIDS and STD education among adolescents is important. The study aimed to assess late adolescents’ knowledge about AIDS and attitudes towards sexually transmitted diseases (STD) among late adolescents. Descriptive comparative study was conducted from May to July 2017 using a sample of 300 students from 03 purposely selected schools in Gampaha Municipality area. Those were one boys' school and two girls' school consisting of students from all 04 subject streams recommended for G.C.E. Advanced Level. They were given a self-administered semi-structured questionnaire. First part consisted of demographic features and second part consisted of close-ended questions with one open-ended question under five topics. Data was analysed using descriptive statistical method, SPSS Software. The sample consisted of 48.6% boys and 51.4% girls. The knowledge related to AIDS was extremely satisfied (92%). The knowledge related to STDs was less satisfied (45%). Knowledge related to STDs was significantly high in bio stream students (87%). Attitudes towards AIDS and STDs were extremely satisfied (100%). There was no significant difference of knowledge and attitudes regarding STIs/HIV in male and female late adolescents. The awareness of late adolescents regarding AIDS/STDs is extremely satisfied. But this awareness has not necessarily translated to practices and behaviours due to certain barriers. This study shows that there is a need for disseminating appropriate information, to use education and communication strategies for increasing the awareness of STIs / HIV.

    Efficacy of a prepared traditional lepa for the management of Amavata sandi shotha

    Amavata is a disease caused by vitiation of Vata by Ama, which is the product of incomplete digestion and metabolism. Agni in Ayurveda includes the digestive processes of gastrointestinal tract as well as the metabolic processes in tissues. Ama could therefore be produced by flawed digestion in gastrointestinal tract or in the tissues by defective metabolic process. Ama thus produced is said to circulate and block the vessel of the joints and get deposited there causing inflammation by producing symptoms such as pain, moving discomfort, swelling and morning stiffness in small and big joints, making a person lame. Various drug trails were already carried out on Amavata, yet there is a lacuna in the management of Ama vata. Therefore this study was undertaken to examine the efficacy of a prepared traditional lepa in the management of Amavata sandi shotha. Hence, in the present clinical study, 30 patients were selected and divided into 2 groups, Group A and B. Group A was given Sarshapa oil and group B the test group was given, traditional lepa. The collected data was recorded according to the age, sex and a paired sample t – test was applied for the clinical assessment of the subjective parameters. During treatment no side effects were noted. According to the analysis all P- values of the test group in the comparison of before and after treatment in full duration were less than 0.05 (P - values before and after treatment on pain (0.000), moving discomfort (0. 000), edema (0. 000), redness (0.012), burning sensation (0.000), ESR (0.010) and Rheumatoid Factor 0.025). The results obtained were significant statistically in traditional lepa. Therefore, the study confirms that the traditional lepa is effective in the management of Amavata sandishotha

    Series of case studies on the efficacy of traditional treatment modality in the management of Carpal Tunnel Syndrome.

    Carpal Tunnel Syndrome or median neuropathy at the wrist is a disabling condition present in3.8% of the general population. The condition is characterized by the compression of median nerve at the wrist leading to paresthesia, numbness, pain and muscle weakness in the hand. Treatment in allopathic medicine includes the use of anti-inammatory drugs, diuretics and surgery which may lead to persistence of symptoms for prolonged period or the recurrence of the symptoms. Therefore, it requires some therapy which can cure the syndrome with minimum intervention; Ayurvedic principles and management play a vital role in the treatment of Carpal Tunnel Syndrome. Thirty patients with Carpal Tunnel Syndrome were selected randomly irrespective of their age, gender, chronicity, occupation etc. and treated with Nikadi TailaAbhyanga, Nikadi Patra Pottali Sweda followed by application of Siyabaladi Lepa mentioned in traditional medicine, daily for a period of one month. The assessment was carried out on the basis of relief found in the cardinal signs and symptoms of the disease adopting Likert scale, depending upon their severity. The clinical trial showed remarkable reduction of pain, numbness and tingling sensation which were found to have p value of < 0.05 which was statistically signicant. Therefore, it can be concluded that the selected treatment modality is effective and safe in the management of Carpal Tunnel Syndrome.

    An analytical study on the antimicrobial activity of Visarpahara Tailaya against Candida albicans and Staphylococcus aureus.

    Visarpahara Tailaya is a herbo-mineral preparation recommended to use clinically in the management of eczema, scabies, itches and other types of skin diseases mainly caused by bacteria and fungi. This study was conducted to determine the antimicrobial activity of this drug using the standard Antimicrobial Sensitivity Test against Staphylococcus aureus and Candida albicans. The test was performed according to the Well diffusion Method having 6 mm diameter wells on Mueller Hinton Agar (MHA) for S. aureus and Sabouraud Dextrose Agar (SDA) for C. albicans. As the positive controls, Amoxicillin 10mg/ml was used for S.aureus and Fluconazole 2.5mg /ml for C. albicans. According to the results, the test drug showed an average Inhibition Zone Diameter (IZD) of 12mm for S. aureus and 17mm for C. albicans while the positive controls showed 51mm and 25mm respectively .These results show that the test drug Visarpahara Thailaya is effective for both test organisms with an increased effectiveness for C.albicans compared to S. aureus. Therefore, it is concluded that Visarpahara Tailaya can be effectively used in the management of C. albicans while it is less effective for S. aureus.

    Jalaukavacharana and wet cupping in the management of sciatic nerve compression: a comparative clinical study.

    The classically sciatic pain is distributed in pathway of sciatic nerve. Herniated intervertebral disc is believed to cause most cases around 90%. Other lesions affecting the integrity of the lumbosacral nerve roots (L4-S3) or the sciatic nerve include lumbar canal or foramina, stenosis, tumors, cysts, hemorrhages, abscess, fractures and some more uncommon conditions may produce same clinical features. The characteristic condition of sciatica is most related to Gudhrasi mentioned in Ayurveda. Gudhrasi cikitsa is considered under Vatavyadhi cikitsa. Chakradatta mentioned that blood letting is effective in the management of Vatavyadhi. Jalaukavacharana is one of the common methods used for blood letting in Ayurveda. Wet cupping is also a procedure of blood letting related to Alabu mentioned in Ayurveda. A comparative clinical study was designed with 20 patients of Group A being treated with Jalaukavacharana alone and 20 patients of Group B with wet cupping alone. The data were analyzed individually with Sciatica Bothersomeness Index (SBI) and Sciatica Frequency index (SFI) and compared using contingency coefcient and two way ANOVA. The results show signicant difference between the groups with P value less than 0.001. Wet cupping group (B) showed highly signicant results dominating over Jalaukavacharana group (A). Though both Jalavkavacharana and Wet cupping provided relief, Wet cupping provided an acute and sustained relief.

    A clinical study to evaluate the efficacy of selected treatment modality in the management of Vata kantaka

    Walking in irregular or uneven ground, structural deformity of foot, excessive strain on heels etc. lead to vitiation of Vata dosha. Aggravated Vata is lodged in ankle joint, especially in dependent parts like heel and hence the pain is caused due to reduced strength of muscles and joints. ‘Kantaka’ refers to thorn. The pricking pain felt due to vitiated Vata is termed as Vata kantaka. This condition is usually seen in the case with calcaneus spur. Symptoms include pain in heel and surrounding region. Pain is more evident soon after waking up in the morning and when the patient stands up after a long rest, or stands for long time. According to previous research (Planter) heel pain is the most prevalent complaint to presenting foot and ankle specialists and may be seen in upwards of 11 % to 15% of adults. So it is the need of hour to focus effective practices which yield good results. In the present study, thirty patients were selected and treated with Pinda Thaila Abyanga, Rasnadi sweda followed by application of Hinguadi lepa mentioned in traditional medicine, daily for one month. The assessment was carried out on the basis of relief found in the cardinal signs and symptoms of the disease adopting scoring, depending upon their severity. Significant reduction in pain was seen at the end of the study with p value of < 0.05. Also there was reduction in other symptoms such as cracks, pruritus, burning sensation and laxity of skin over the foot. There were no adverse effects reported during the study. Therefore, it can be concluded that the selected treatment modality is effective in the management of Vata kantaka.

    A clinical study to evaluate the efficacy of Elaeocarpus serratus and Psidium guajava leaves paste in the management of Dandruff

    Darunaka is a disease concerned with hair root which can correlate with Dandruff, which is the most common cause for hair loss. It is more of a social problem and affects aesthetic value as a person with Dandruff is quite likely to feel down on the social ring. Acharya Sushruta has mentioned Darunaka under Kshudraroga’s with symptoms like rukshata, kandu, twak sphutana etc. Dandruff can occur due to air pollution, water pollution, changed life style, irregular daily regimen, poor hygiene and immune system, sweating, mental stress etc which can lead to several bacterial and fungal infections. It is a common scalp disorder affecting almost half of the population at the pre-pubertal age and of any gender and ethnicity. No population in any geographical region would have passed through freely without being affected by dandruff at some stage in their life. Shiroabhyanga, Nasya, Shirodhara, Shirobasti, Shirolepa and Siravyadha is the line of treatment for Darunaka in the classics. In traditional medicine, paste of leaves of Elaeocarpus serratus and Psidium guajava have been used for thousand years as a home remedy for dandruff. In spite of the present medical solutions, Dandruff relapses quite often. A permanent and holistic solution is the need of the hour. Hence, this work is intended to study the efficacy of Elaeocarpus serratus and Psidium guajava leaves paste for dandruff. Thirty patients were selected who were advised to apply the paste covering whole scalp area for one hour and rinse it, for a period of one month every alternative day. Signs and symptoms were assessed by using grading system and the significant reduction of symptoms was observed. Accordingly, more than 40% patients gained marked improvement. No adverse reaction was reported in this study period. It is concluded that Elaeocarpus serratus and Psidium guajava leaves paste is effective in the management of Dandruff.
